Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
Health Insurance Plans
We participate in Medicare, most other insurance plans, and many managed care and HMO’s. Always bring your insurance card(s) for each appointment. Always keep us informed of any changes to your phone number, address or insurance. Covered procedures will vary with each insurance company — our office staff will assist you in determining eligibility. Patients are responsible for deductibles, co-payments and remaining balances.
Methods of Payment
We will bill all primary and secondary insurances for you if you provide the insurance information. Co-payments are expected to be paid at the time of service. The financial responsibility for services rests with the patient or his/her family, regardless of your insurance coverage. We also accept the following credit cards:
Updating Records
It is very important that your doctor have your up-to-date address and phone number. In case of an emergency we must be able to contact you. Please make sure the receptionist is aware of any changes. Also, please make sure we have both work and home phone numbers and another emergency contact number if possible.
